Could not find anyone in three sources I have checked. I would think that possibly by the time a recipient of a DFM came up for another award he would be an officer and receive a DFC ?
Good question! Come to think of it I can't recall ever seeing DFM and bar either...and I think your logic is correct : anyone competent enough to win a DFM would rise in rank quickly. Interested to hear what you come up with.

Yes it appears that the RAF had a number of DFM with bar recipients.And the RCAF did not seem to have any that can be found, I know from my research over the last six years on 419 Squadron the RAF was slow at commissioning airmen. On many crews that started with all Sergeants after a while many were officers while the RAF F/E left as the same rank. maybe that is the reason.
"competent" that was the word I was trying to find when I wrote that question. As i say below I think RCAF were quicker at giving out commissions and as you say a competent airmen would have been soon an officer.
Here is an odd auxiliary question : What is the LOWEST rank of any pilot completing a tour? Two tours? ie did any pilot complete one or two tours , staying the rank of Flt Sgt?
